Durante los últimos años ha sido evidente a los investigadores de que la hiperglicemia, o aún una simple anomalía poco importante de la curva de glicemia, ya es hallazgo tardío en el desarrollo del estado diabético.
Se ha comprobado el concepto de Naunyn sobre la índole hereditaria de esta enfermedad. La naturaleza del cambio genético todavía se desconoce pero lo que si sabemos es que el individio con diabetes posee uno o más genes alterados. Según lo anterior, la diabetes es una "enfermedad molecular" en la cual importantes "componentes" celulares faltan, están severamente perturbados o se han perdido.
