Abstract

Adrenocortical carcinoma (CAC) is a rare malignant tumor. The clinical presentation is usually due to excess hormonal production. The involvement of the venous system with thrombi in the inferior vena cava (IVC) is only reported in a third of all cases. This study reports a case of CAC with hormonal production which presents as tumor thrombus extending to the IVC and right atrium, in association with pulmonary embolism. This is a 63-year-old female patient with a recent diagnosis of systemic hypertension, who is admitted to the emergency department for abdominal pain, nausea, vomiting and weight loss. Conventional images reported an unresectable mass in the right adrenal fossa not ammenable to surgical treatment. Pathology was used to diagnose an adrenal cortex neoplasia with extensive necrosis. Mitotane and outpatient chemotherapy scheme were started.
