Abstract

Context: Biopsychosocial factors influence therapeutic adherence in patients with diabetes mellitus 2.

Objective: was to determine adherence to pharmacological management and its association with biopsychosocial factors.

Methodology: Observational, cross-sectional study with an analytical component. Patients older than 18 years with type 2 diabetes hospitalized for any cause who signed the informed consent were included. The MOS, PHQ4, DDS-17, and Bonilla y de Reales scales were used.

Results: A total of 87 patients were included, with a mean age of 61 years, 56.3% were men, 59.7% had BMI > 25 and 72% had polypharmacy, 16.1% had a poor support network, 31% and 27.9% had probable depressive and anxiety disorder respectively, 51.7% had high stress due to emotional burden and 49.4% stress related to the therapeutic regimen. 58.6% had a risk of non-adherence and 14.9% non-adherence behavior. The median of therapeutic adherence was 0.95 and of persistence 0.9. A significant association was found between non-adherence behavior with stress, potential cases of depression, poor support network, history of neuropathy, and peripheral arterial disease. Finally, a significant relation was observed between high adherence rate and age > 60 years, BMI > 25, polypharmacy, high comorbidity, heart failure, kidney disease, and retinopathy.

Conclusion: There is better adherence in patients older than 60 years, with BMI > 25, polypharmacy, and high comorbidity, with a good support network and absence of stress or depression. Biopsychosocial factors should be evaluated in all patients with type 2 diabetes.
