Abstract

Background: Thyrotoxicosis is the syndrome caused by excess free thyroid hormones, interrogation and physical examination are useful for the etiological diagnosis, it can often be difficult to establish without imaging, thyroid scintigraphy and ultrasound are necessary tools in confirmation of cause of thyrotoxicosis, however the imaging findings can be controversial and discordant.
Purpose: The use of diagnostic images such as thyroid Doppler ultrasound and scintigraphy are very useful in thyrotoxicosis, however, they must have a close correlation with the natural history of the disease and clinical findings.
Cases presentation: 23-year-old woman with palpitations, sweating, 3-month weight loss, scintigraphy that reports mild hypouptake of the left lobe compatible with subacute thyroiditis; thyroid ultrasound with a highly vascular nodule in the right lobe; final diagnosis of thyrotoxicosis due to toxic adenoma. A 41-year-old man, 3 months of evolution, with a painful mass on the left paratracheal anterior aspect of the neck, associated with palpitations, weight loss, hair loss, conciliation insomnia and dyspnea; scintigraphy that reports left toxic adenoma, ultrasound with areas markedly hypoechoic in both lobes, in patches, with loss of vascularization, final diagnosis of subacute thyroiditis.
Discussion and conclusion: the use of diagnostic images such as thyroid Doppler ultrasound and scintigraphy are very useful in thyrotoxicosis, however, they must have a close correlation with the natural history of the disease and clinical findings.
