Gout is an inflammatory arthritis caused by excessive accumulation of monosodium urate crystals at the tissues, with joint involvement predominantly. Its clinical manifestations are varied, usually conditioning a chronic course of the disease. The diagnosis is made through the set of clinical, biochemical and imaging findings. The treatment is based on non-pharmacological and pharmacological measures within which xanthine oxidase inhibitors and uricosurics are described. A case of a male patient with gout of difficult management and refractory hyperuricemia is described.
